雾化吸入沙丁胺醇用于哮喘治疗:60min时对血钾和血磷浓度的影响

目的:我们开展此项前瞻性研究,旨在扩展急性哮喘治疗时血清磷酸盐的水平相关的可用资料。本研究使用,β肾上腺素激动剂沙丁胺醇作为哮喘治疗药物。
资料和方法:纳入26例符合条件的患者,纳入标准:年龄>16岁,有哮喘病史,患者处于急性加重期。所有患者血尿素氮、肌酐和血糖水平在正常范围内。正在接受长效茶碱治疗的患者不入选。检测基线血磷和血钾水平。给予患者雾化吸入沙丁胺醇(2.5mg),每小时三次。60min后,检测血磷和血钾水平。
结果:血磷水平从基线的3.7±0.9mg/dL降至60min时的3.6±0.9mg/dL,无统计学差异(p=0.373)。血钾水平从基线的4.6±0.7mmol/L降至60min时的4.3±0.7mmol/L,差异具有统计学意义(p<0.001)。
结论:哮喘急性加重期急诊治疗时给予雾化吸入沙丁胺醇治疗不会导致血磷水平显著降低,但可导致明显的低钾血症。该研究表明,需进一步研究阐明雾化吸入沙丁胺醇对血磷水平是否有显著临床影响。

Nebulized salbutamol for asthma: Effects on serum potassium and phosphate levels at the 60min.

Abstract
OBJECTIVE: We conducted this prospective study to expand available information in relation to serum phosphate levels in treatment of acute asthma. A β-adrenergic agonist, salbutamol, was used for this purpose.
MATERIAL AND METHODS: Twenty-six patients who met the inclusion criteria as; age over 16 years, asthma history, and an acute exacerbation were included. Serum blood urea nitrogen, creatinine, glucose were within normal limits in all the patients. None of the patients were on chronic theophylline therapy. Baseline serum phosphate and potassium levels were measured. Nebulized salbutamol (2.5mg) was used three times at every hour. After 60min, serum phosphate and potassium levels were measured.
RESULTS: Serum phosphate levels decreased from 3.7±0.9mg/dL (baseline) to 3.6±0.9mg/dL at 60min. This decrease was not statistically significant (p=0.373). Serum potassium levels decreased significantly (p<0.001) from 4.6±0.7mmol/L (baseline) to 4.3±0.7mmol/L (60min).
CONCLUSION: Administration of nebulized salbutamol during the emergency treatment of acute exacerbation of asthma is not associated with a statistical decrease in serum phosphate. There was significant hypokalemia. This study indicates that a further study is needed to elucidate the clinical significance of nebulized salbutamol on serum phosphate.
