多个工具可用于检测儿童哮喘控制水平。本试验旨在比较GINA指南、儿童哮喘控制测试(C-ACT)和哮喘控制测试在检测儿童哮喘控制中的差异。145名哮喘儿童入选本研究,并填充以网络为基础的每日饮食卡,连续4周,主要调查哮喘症状、急救用药、活动受限,随后进行C-ACT和ACT检测。C-ACT和ACT中采用截止值为19来预测未控制哮喘。基于GINA标准,71名儿童(51%)哮喘未控制,19名(14%)儿童哮喘完全控制。与GINA指南相比,C-ACT和ACT的受试者工作曲线(ROC)的曲线下面积(AUC)分别为0.89和0.92。C-ACT和ACT采用19作为截止值时,其预测哮喘未控制的敏感性分别为33%和66%。在预测哮喘未控制上,C-ACT和ACT与GINA标准具有很好相关性,但如果采用常用的C-ACT和ACT截止值,获得的哮喘未控制儿童比例要低于GINA评价的哮喘未控制儿童比例。

Abstract
Several tools are useful in detecting uncontrolled asthma in children. The aim of this study was to compare GINA guidelines with the Childhood Asthma Control Test (C-ACT) and the Asthma Control Test (ACT) in detecting uncontrolled asthma in children. 145 children with asthma filled in a web-based daily diary card for 4 weeks on symptoms, use of rescue medication and limitations of activities, followed by either the C-ACT or ACT. For predicting uncontrolled asthma, cut-off points of 19 for C-ACT and ACT were used. According to GINA guidelines, asthma was uncontrolled in 71 children (51%) and completely controlled in 19 children (14%). The area under the curve (AUC) in the Receiver Operating Characteristic (ROC) curves for C-ACT and ACT versus GINA guidelines were 0.89 and 0.92, respectively. Cut-off points of 19 for C-ACT and ACT resulted in a sensitivity of 33% and 66% in predicting uncontrolled asthma, respectively. C-ACT and ACT correlate well with GINA criteria in predicting uncontrolled asthma, but commonly used cut-off points for C-ACT and ACT seem to underestimate the proportion of children with uncontrolled asthma as defined by GINA.
