肺功能检测对哮喘患者支气管和肺泡中一氧化氮浓度的影响

   摘要
   目的:50 ml/s恒定流速测定FENO的研究结果一直是模棱两可的。另外,肺功能检测对临床和研究中使用越来越多的两种方法,即支气管 NO流量(J'awNO)和肺泡NO (CANO)的作用仍然不明。本研究旨在评估呼吸量测定对成人哮喘患者FENO, J'awNO 和CANO的作用。
   方法:44例成人哮喘患者入组。评估呼出气NO测定本身对呼出气NO值的影响,FENO, J'awNO 和 CANO 基线时间点和休息10分钟后分别测定一次。肺功能检测(应用/不应用支气管扩张剂)后10分钟进行呼出气NO测定。
   结果:支气管扩张剂使用前组(n = 26),肺功能检测前,FENO中位值(95%可信区间)为37.3 ppb (22.2-52.4),J'awNO 为2375 pl/s (1613-3137), CANO为1.65 ppb (0.95-2.35),肺功能检测后这些值分别为35.5 ppb (21.1-49.0, P=0.10), 2402 pl/s (1663-3141, P=0.85) 和1.60 ppb (0.64-2.56, P=0.87)。肺功能检测引起的呼出气NO含量变化在支气管扩张剂使用前、后组间没有显著性差别(n=18)。同时,两组FENO, J'awNO 和CANO 值相似。
   结论:我们发现肺功能检测(用/不用支气管扩张剂)不会引起支气管NO流量或肺泡NO含量的显著变化。因此,呼出气NO值可以在测定肺功能后获得。

The effect of spirometry on bronchial and alveolar nitric oxide in subjects with asthma.

Abstract
OBJECTIVE:
The effect of spirometric maneuvers on exhaled nitric oxide (NO) at the constant flow rate of 50 ml/s (FENO) has been studied with equivocal results. Furthermore, the effects of spirometry on bronchial NO flux (J'awNO) and alveolar NO (CANO), two measurements increasingly being used in clinical and research protocols, are unknown. The aim of this study was to evaluate the effect of spirometry on FENO, J'awNO and CANO in adults with asthma.
METHODS: Forty-four adults with asthma were studied. To assess the impact of exhaled NO measurement itself on exhaled NO values, FENO, J'awNO and CANO were obtained twice, at baseline and after a resting period of 10 min. Then spirometry (with or without bronchodilator) was performed followed by exhaled NO measurements at 10 min.
RESULTS: In the group with pre bronchodilator study only (n = 26), mean (95% CI) values before spirometry were 37.3 ppb (22.2-52.4) for FENO, 2375 pl/s (1613-3137) for J'awNO and 1.65 ppb (0.95-2.35) for CANO, compared to 35.5 ppb (21.1-49.0, P=0.10), 2402 pl/s (1663-3141, P=0.85) and 1.60 ppb (0.64-2.56, P=0.87) after spirometry, respectively. Spirometry-induced changes in exhaled NO values were also no significant in the group with both pre and post bronchodilator study (n = 18). Furthermore, changes in FENO, J'awNO and CANO values were similar in the two groups.
CONCLUSIONS: Our findings demonstrate that spirometry (with or without bronchodilator) does not induce significant changes in bronchial NO flux or alveolar NO values. Therefore, exhaled NO values may be obtained after spirometric
