Associations of serum cotinine with asthma risk in US children: a mediation analysis of body mass index.

Abstract
BACKGROUND:In recent years, the prevalence of asthma among children has increased, with tobacco smoke exposure identified as a major contributor. However, the effects of tobacco exposure on childhood asthma remain insufficiently investigated.
OBJECTIVES:This study aims to investigate the relationship between serum cotinine, a biomarker of tobacco exposure, and the risk of childhood asthma, and to explore whether body mass index (BMI) mediates this relationship.
METHODS:Using data from the National Health and Nutrition Examination Survey (NHANES) from 2011 to 2018, this study analyzed serum cotinine, physician-diagnosed asthma and BMI in children aged 3-12 years. Weighted multivariable logistic regression models, subgroup analyses, smooth curve fitting, threshold effect analysis and mediation analysis were employed to examine the relationship between serum cotinine and asthma.
RESULTS:The study found a non-linear relationship between serum cotinine and asthma. The risk appeared to reach saturation when the Log2-serum cotinine value was -0.3 (equivalent to serum cotinine concentration of 0.81 ng/mL). Additionally, mediation analysis revealed that BMI accounts for approximately 12.21% of the mediating effect in the relationship between serum cotinine and increased asthma risk.
CONCLUSION: The present study indicates that elevated serum cotinine, a novel biomarker of tobacco exposure, are significantly associated with an increased risk of childhood asthma and that BMI plays a significant mediating role in this relationship.
